Bug in NTS + pool

There is a bug in the code I pushed yesterday.  If one of the new slots 
runs out of cookies, something screws up and we get a new slot rather than 
refilling the existing one.

pool cloudflare nts is a wonderful test case.  They have a bug in their 
current code that drops the current key for decoding cookies when they do 
a key rotate.  They rotate every 6 hours starting at midnight UTC.
